The purpose of this study was to review our experience with the management of this common problem in the intensive care unit (ICU) and to assess the impact of early interventions on patient outcome. Over a 2.5-year period, all patients who presented with acute upper gastrointestinal bleeding and required the care of a surgical ICU were retrospectively reviewed. All patients had an upper endoscopy performed within the first 72 h of ICU admission to exclude gastrointestinal bleeding. Upper endoscopy was performed within 24 h in patients with active bleeding. The outcome of interest was mortality. Forty-five patients met the inclusion criteria. Three of these patients (6.7%) had active bleeding at the time of ICU admission. All 45 patients were evaluated by upper endoscopy. Two patients required immediate endoscopy for active bleeding. None of the patients who required emergency endoscopy for active bleeding died. Forty-five patients (100%) had either no bleeding or had bleeding controlled with endoscopic intervention. Thirty-eight patients (84.4%) died. Eleven (24.4%) patients survived. Overall mortality was significantly higher in patients with active bleeding (p = 0.03). Although there was no difference in the mortality rate in patients who required urgent endoscopy and those who did not (p = 0.78), the survival rate was significantly higher in patients who underwent early endoscopy (p = 0.02).
